If a move to a new address is one of the changes your family underwent in 2018, be sure you’ve shared it with us! Several of our holiday cards were returned to us because we didn’t have current addresses.
The same applies to updated phone numbers, email addresses and any other demographic information – especially insurance cards! We send the information in your medical chart with your referrals, prescriptions and samples prepared for labs so you get the full benefit of your insurance plan.
